Medications
To refill a medication, please call our office and leave a message with the nurse with the name of the medicine, the dosage, and your pharmacy. You can also send a refill request through the patient portal.
24-Hour Policy: All requests for prescription refills will be available at the time of request on the next business day.
Appointments
- We have a 15-minute late policy. If you arrive to your appointment 15 minutes or later, you will have to reschedule.
- New patients to our practice must first be seen for an establishing appointment. These visits are typically only available several weeks or months out, so please schedule as soon as you are able.
- For this first appointment, we ask that you bring your insurance card, a photo ID, the bottles of any medicines you are regularly taking (prescription and over-the-counter), and be prepared to pay your copay. Click here for a list of insurers with which we participate.
- It is our policy that if you “no show” for your first appointment (missing your appointment without calling to cancel or reschedule) we will not reschedule your appointment. If you cannot make it to your first appointment, please call to reschedule.
- If you are unable to make your appointment, please call us 24 hours prior to your scheduled time to cancel. If you do not come to your scheduled appointment without calling to cancel for a total of three times, we may have no alternative than to direct you to another physician or practice for medical care.
- If you find it difficult to keep your appointment (due to transportation, financial difficulties, etc.), please call us and ask to speak with a care manager. They may be able to provide the help you need.
Payment
We collect copays at the start of your visit. We cannot bill you for your copay; this must be paid at the time of your visit. For self-pay patients, we collect an initial fee up front, and if any other procedure are done (urinalysis, PFT, etc), the rest is collected at check out. If you have a question about what a visit might cost, please call our office. We accept Visa, Mastercard, and Discover card for payments, as well as cash and checks. We also accept CareCredit as a form of payment.
While payment is expected at the time of service, we offer payment plans and reduced fees for those in financial need. If you have any concerns about being able to pay for your visit, please ask to speak with Doris. For more information, read this brochure.
Referrals and Pre-Authorizations
Referrals must be requested at least 2-3 business days prior to your appointment. Pre authorizations must be requested at least 5 business days prior to your appointment. Referrals cannot be faxed to doctor’s offices; they must be picked up at our office. Failure to do so may result in rescheduling of the appointment.

Medical Records
If you need to request a copy of your records or to transfer records we will need a signed release. This form is available our office or at any other doctor’s office. Please call our office with any questions.
Tests
We are able to perform some diagnostic testing, including electrocardiograms (EKG), pulmonary function tests (PFT), urine analysis, audiometry (hearing test), and in-office hemoglobin A1c. Labs can be done with the LabCorp phlebotomist in our office, or choose another location.
Our office will contact you with the results, either by phone, letter, or through the patient portal. If you don’t hear from us in a timely manner, please call our office and we will get back to you with those results.
Forms
Any form that is required to be filled out that is not the sole purpose of an office visit will be charged a $15 fee (cash/credit card/check accepted). Anyone requesting a dictated or typed letter will be charged a $15 fee.
Motor Vehicle Accidents
Visits regarding motor vehicle accidents are self-pay; we do not submit bills to car insurance. We will provide a receipt you can submit to your car insurance for reimbursement.
Worker’s Comp
If you were injured at work, please let us know when you schedule that the visit is for a work injury. Come to your visit early with your claim number and worker’s comp carrier information ready; there is some paperwork you will need to complete before your appointment.
