Notice to patients of Dr. Neil Lattin:
Dr. Neil Lattin is retiring on December 31, 2024, after which, Stone Run Family Medicine will be maintaining his medical records. Requests for medical records may be made with our practice. Requests can be faxed to (410) 658-4548, mailed to 101 Colonial Way, Ste A, Rising Sun, MD 21911, or dropped off at our front desk. Requests must be signed by the patient or patient’s legal representative and dated. Download the release form here. For any billing related questions, please call (443) 452-3707, extension 102.
Please note: Patients of Dr. Lattin who would like to be seen as a patient at Stone Run Family Medicine need to call our front desk and schedule a new patient appointment. We will not be able to provide any medical care to Dr. Lattin’s patients unless they establish as a new patient with Stone Run. Please refer to the list of insurers we participate with before calling.
