If you need a prescription refilled, please call our office and ask for the nurse's
line, where you may leave a message with the name of the medicine, dosage, and pharmacy, or send a refill request through the patient portal. 24-Hour Policy: All requests for prescription refills will be available at the time of request on the next business day.
All prescriptions that need to be picked up must be picked up by the patient, or if the patient is under the age of 18, their parent or guardian. If circumstances prohibit the patient to pick up their own prescription, it will be at the office's discretion.
Any prescription that is a Schedule II or Schedule III drug needs to be picked up at our office. We can no longer send these electronically to the pharmacy.
- We have a 15-minute late policy. If you arrive to your appointment 15 minutes or later, you will have to reschedule.
- It is our policy that if you "no show" (don't show up for your appointment without giving us
advance notice) for your first appointment, we will not accept you into our practice. If you cannot
make it to your first appointment, please call to reschedule.
- If you are unable to make your appointment, please call us 24 hours prior to your scheduled time to cancel. If you do not come to your scheduled appointment without calling to cancel it for a total of three times, we may have no alternative than to direct you to another physician and/or clinic for medical care.
If you find it difficult to keep your appointment (for example: not having transportation), please call us and ask to speak to a care mananger. They may be able to provide the help you need.
We collect copays at the start of your visit. We cannot bill you for your copay; this must be payed at the time of your visit. For self-pay patients, we collect an initial fee up front, and if anything else was done (urinalysis, PFT, etc), the rest is colleced at check out. If you have a question about what a visit might cost, please call our office. We accept Visa, Mastercard, and Discover card for payments, as well as cash and checks.
While payment is expected at the time of service, we offer payment plans and reduced fees for those in financial need. If you have any concerns about being able to pay for your visit, please ask to speak with Doris. For more information, read this brochure
Referrals and Pre-Authorizations
Referrals must be requested at least 2-3 business days prior to your appointment. Pre
authorizations must be requested at least 5 business days prior to your appointment. Referrals cannot
be faxed to doctor's offices; they must be picked up at our office. Failure to do so may result in
rescheduling of the appointment.
To request a referral, call our office and ask for Kate's line, where you can leave a detailed message about your appointment. You can also fill out a referral request, available at here or at our front desk.
We are able to perform some diagnostic testing, including electrocardiograms (EKG), pulmonary function tests (PFT), urine analysis, and bone density scans (DEXA). For any labs or x-rays, you can use the office in the suite next to ours (Rising Sun Diagnostics) or choose another location.
Our office will contact you with the results, either by phone, letter, or through the patient portal. If you don't hear from us in a timely manner, please call our office and we will get back to you with those results.
Any form that is required to be filled out that is not the sole purpose of an office visit will be charged a $15 fee (cash/credit card/check accepted). Anyone requesting a dictated or typed letter will be charged a $15 fee.
